Name - Obvious Enough
Age - Again, Obvious
Zodiac Sign - This can be filled out even if the character is not an actual Zodiac Warrior but just a soldier in one of the Zodiac armies.
Soldier or Zodiac Warrior - A simple question. Is your character one of the army soldiers or are they an actual Zodiac warrior? Note that in order to be a Zodiac Warrior, your character must only be born in the time frame of the Zodiac they embody. Example, a Capricorn must be born between December 22 and January 20. A person is chosen by the gods to be a Zodiac Warrior seemingly at random. The revelation is often than a divine messenger will appear at their home to ask them to come away and train to become a Warrior of the Zodiac. These individuals must agree, however. If they do not, then the gods move on to find another. Soldiers are different. Soldiers join the Zodiac armies based on their own choice. However, they are narrowed down by the gods in a test to find the most fit soldiers for battle. Only the best in battle against Titans.
Preferred Weapon - Basically asking what weapon/weapons your character may use most often.
Training Extent - Beginner, Novice, Average, Intermediate, Advanced, Elite/Training Complete? Which of those would your character be? This applies both to Soldiers and Zodiac Warriors.
Ambitions - What are some things your character hopes to accomplish? It doesn't have to be simply something they hope to accomplish as a part of joining the Zodiac or Zodiac Armies but what they generally hop to do with their lives.
Likes - What are some things your character likes or likes doing?
Dislikes/Fears - What are some things your character dislikes or is afraid of.
Appearance Description or Image - An image or description of how your character may look. Keep in mind that the time-frame is semi-medieval.

You are also allowed to create NPC's. NPC's have no concrete sign-up application. You may create as many as you want in any mold you see fit. These are created dominantly for your own story arcs 9 times out of 10.

Dislikes/Fears: He has always hated when one of the passing through students would bully or pick on him, and has always wanted to stand up to them
Appearance Description or Image: Shoulder length black hair, suprisingly pale skin for all the time he spends outside, usually wears a black cloak, loose black trousers, and a plain shirt. Eyes are deep blue that can convey all the hope and wonder int he world, but he has never been close enough to anyone to look at them that way, as a friend.
